Case study: Jaundice in Pregnancy

A 22 Yrs old female admitted in the Synergy Ms Hospital on 15/12/2022 who was referred from outside. The Patient was having G3A2 with 9 mA with at the time of admission and was in semi conscious stage, drowsy, disoriented to surroundings with Icterus.
The patient attended by Dr. Rinaz Patel under the guidance of Dr. Ravindra Aarli and Dr. Vinaya Kulkarni.
Status - Per abdomen examination- Foetal Hear Sound (FHS) absent patient vitally stable.Adviced Ultrasonography (USG=IUD Baby)
Induction of Labour done.
Due to contractions patient became more irritable.
Input output was not Adequate.
Fluids were given according to advised by physician.
Normal Delivery done (FTND)
Bleeding ++, Vaginal packing done Removed after 12 hr.
During Delivery Pt. was too Rowdy & toxic to handle, but Synergy team handled & conducted the delivery very well.
Now patient is under observation in ICU.
Post Delivery Status-
Bleeding present + Hemostatic Management with FFP on. Patient is vitally stable, conscious, well oriented and on oral intake. Patient Reports are Improving.
Diagnosis-
G3A1 & 36 ks of POG & baby & Higher Bilirubin & High serum creatinine delivered vaginally & Vertex.
Remarks-
The incidence of jaundice during pregnancy was seen to be 1 in 429 patients.
The incidence of jaundice in India varies from 0.4 to 0.9/1000 Deliveries.
Jaundice during pregnancy was 2.4 per 1000 live births.
